Lottery Application for students entering GRADES K-8 at Explore Academy - Peoria for 2024-2025
This application indicates interest in enrolling your student at Explore Academy - Peoria for the 2024-2025 school year. Please do not submit more than one application per student or your enrollment spot may be altered.

Explore Academy has partnered with Freedom Prep to expand its grade offering to include grades K-5. Families who apply for grades K-5 will be enrolling with Freedom Prep officially, although the academic program will be at Explore Academy.

A.R.S. § 15-802(B) requires school districts and charter schools to obtain and maintain verifiable documentation of Arizona residency upon enrollment in an Arizona public school. This documentation must be provided at initial enrollment of a student in a school district or charter school in this state. Every school district or charter school is also required, within 30 days of enrollment, to obtain a certified copy of a pupil’s birth certificate or other reliable proof of the pupil’s identity and age, or a letter from the authorized representative of an agency having custody of the pupil pursuant to title 8, chapter 2 certifying that the pupil has been placed in the custody of the agency as prescribed by law.

In addition, immunization (shot) records (OR an approved exemption form) are required in order to attend school. AZ Immunization Requirements and Exemptions Forms (in both English and Spanish) can be obtained at this link.  Evidence of compliance regarding immunizations must be provided as part of the registration process, once accepted.

If/when your student is accepted via this lottery/enrollment process, you will receive information for completing a registartion packet in the Student Information System (PowerSchool for grades 6-8 and Freedom Prep's System for grades K-5).

Please upload the evidence referenced on the AZ Residency Documentation Form (above) such as a valid Arizona driver's license or Identification card, valid AZ motor vehicle registration, valid AZ Address Confidentiality Program authorization card, mortgage, property tax bill, rental agreement or lease, utility bill (water, electric, gas, cable), bank or credit card statement, W-2 wage statement, or payroll stub. Enrollment will not be complete until the proof of residency is uploaded or delivered to the school office. For parents/legal guardians who are members of the armed services, please provide copy of the document of transfer or pending transfer to a military installation within the state. (In this case, the parent must provide official documentation of residency within ten days after the arrival date which may include a temporary on-base billeting facility as their address.)
